## What is Social360?

A **practitioner-centric, psychosocial case management system** built to maximise the time case workers spend with clients by streamlining essential tasks and eliminating administrative overhead.&#x20;

Unlike [conventional systems](https://guide.social360.gov.sg/blog/why-many-commercial-case-management-systems-fall-short-in-psychosocial-practice) that impose rigid, transactional workflows more suited to administrative case handling, this system is purpose-designed for helping professionals in health and social care. Its focus is on enabling better co-management and seamless care coordination across providers.

## Why are we doing this?

A major challenge in the social-health sector is **fragmented information**. This disconnect exists within individual organisations and across different entities, making it difficult for social workers to deliver seamless care. Social360 aims to bridge these gaps by providing a unified platform for better care coordination, co-management and collaboration.

## Our Vision

We envision **seamless care across health and social services** — where clients can walk into any provider and receive the help they need without unnecessary delays. Relevant information is accessible from the very first visit, ensuring continuity of care.
