Assessments

To create a new psychometric assessment or view the client's assessment history

You can view all assessments conducted for different assessees under the Assessments tab.

To ensure visibility and prevent duplication, it is recommended to complete assessments directly within the system rather than uploading them to the Documents tab.

Quickly find assessments by:

  • Searching by Name.

Add Assessment

To add an assessment, click Add Assessment button in the top-right corner, then:

  • Select an assessment form.

  • Choose the assessee from the Family & Significant Others table.

Psychometric assessment templates are available to all agencies and services.

Filling up the Assessment

  • Fill in the required fields.

  • Add remarks if necessary.

  • Click Submit to save your responses and view the results.

Assessments cannot be saved as drafts, but you can edit them after submission.

View History & Compare Results

  • For specific assessments e.g. Zarit Burden Interview (ZBI), you can view the full history and compare results side by side to track the client’s progress over time.

Incomplete Assessment

If any fields are left blank, scores will not be calculated, and the results table will display 'Incomplete assessment'. You may provide a reason in the remarks section for documentation purposes.

In scenarios where the client declines to participate, you may choose to leave the assessment entirely blank.

Edit / Delete Assessments

To edit/delete an assessment, click the button:

  • Edit - You can make changes to a submitted assessment at any time.

  • Delete - Assessments can be deleted if needed. This action cannot be undone.
