To submit custom-built forms created by services to capture client-specific information
Context: Each service can create custom forms e.g. Intake or Discharge Forms, which users can then complete within a client’s profile.
Forms created are only visible and fillable by users within your service. However, once submitted, they can be viewed by anyone with access to the client’s profile.
You can access all submitted forms—whether completed by you or other caseworkers—under the Form Submissions tab.

Search
Quickly find form submissions by:
Searching by Name.
Submit New Forms
To submit a new form, click the Add Submission button in the top-right corner, then:
Select the type of form you want to complete.

Fill in the required fields.
Click Save as draft if you need to return later.
Click Submit to save your responses.

Edit / Delete Form Submissions
To edit/delete a assessment, click the button:
Edit Submission - You can make changes to a submitted form submission at any time.
Delete Submission - Form submissions can be deleted if needed. This action cannot be undone.
