The Community Partners module allows you to see other partners who are also supporting your clients, making it easier to coordinate care across organisations.

Integration with Care360

Care360 is a platform used by Medical Social Workers (MSWs) across public healthcare institutions. Social360 pulls the details of MSWs who are also actively working with your client from Care360.

• Date of last interaction — Refers to the date the most recent progress note was recorded.

• Primary MSW (indicated by a star beside their name) — A primary MSW is assigned to clients with more complex or long-term needs.

An active MSW refers to one who has written a progress note for the client within the last year.
